Synopsis of Tonsils

The tonsils are two oval-shaped masses of lymphoid tissue located at the back of the throat, on either side of the oropharynx. They are part of the lymphatic and immune systems and serve as a first line of defense against inhaled or ingested pathogens. Tonsils help detect foreign invaders and initiate immune responses by trapping bacteria and viruses and producing white blood cells (especially lymphocytes).

There are three main types of tonsillar tissues:

  • Palatine tonsils: The commonly referred-to “tonsils” at the back of the throat.

  • Pharyngeal tonsil: Located behind the nasal cavity, also called the adenoids.

  • Lingual tonsils: Located at the base of the tongue.

Together, they form Waldeyer’s ring, a circle of lymphatic tissue guarding the entryways of the respiratory and digestive tracts.

Tonsils play a significant role in childhood immunity, though their function decreases after puberty. Common tonsil-related conditions include:

  • Tonsillitis: Inflammation due to viral or bacterial infection (often Streptococcus). Symptoms include sore throat, fever, swollen tonsils, and difficulty swallowing.

  • Chronic tonsillitis: Recurrent or persistent infections that may lead to removal (tonsillectomy).

  • Tonsil stones (tonsilloliths): Hard deposits of debris and bacteria that form in tonsillar crypts, causing bad breath and throat irritation.

  • Enlarged tonsils: Can obstruct airways, contributing to snoring or sleep apnea, particularly in children.

  • Tonsillar cancer: Rare, but usually associated with HPV infection in adults.

Tonsils may be surgically removed if they cause repeated infections or airway obstruction, though they play a protective role early in life.
