Synopsis of Hiatal Hernia

A hiatal hernia occurs when a portion of the stomach pushes upward through the diaphragm into the chest cavity via the esophageal hiatus—an opening where the esophagus passes through the diaphragm. This condition can interfere with normal digestion and increase the likelihood of acid reflux or gastroesophageal reflux disease (GERD).

There are two main types:

  • Sliding hiatal hernia (most common): The stomach and lower esophagus slide up into the chest.

  • Paraesophageal hernia (less common, more serious): Part of the stomach pushes up alongside the esophagus and may become strangulated.

Common symptoms include:

  • Heartburn or acid reflux

  • Regurgitation of food or sour liquid

  • Difficulty swallowing

  • Chest or upper abdominal discomfort

  • Shortness of breath (in large hernias)

  • Feeling full quickly or bloating

Many small hiatal hernias cause no symptoms and are discovered incidentally. Larger hernias or those causing reflux can significantly impact comfort and digestion.

When to see a doctor:
If you experience persistent heartburn, regurgitation, chest pain, or difficulty eating, a healthcare provider may recommend an endoscopy, barium swallow, or manometry to evaluate the hernia. Surgery may be necessary in severe or strangulated cases.
