Synopsis of Labor and Delivery

Labor and delivery is the final stage of pregnancy in which the uterus contracts to deliver the baby and the placenta. It is a complex physiological process involving hormonal signaling, uterine muscle activity, cervical dilation, and coordinated maternal effort. Labor typically occurs between 37 and 42 weeks of pregnancy and may be spontaneous or medically induced.

Labor is divided into three stages:

  1. First stage: Onset of regular contractions to full cervical dilation (10 cm); includes early, active, and transition phases.

  2. Second stage: Full dilation to delivery of the baby.

  3. Third stage: Delivery of the placenta.

Common symptoms and signs of labor include:

  • Regular, increasingly intense contractions

  • Bloody show (mucus plug release)

  • Water breaking (amniotic sac rupture)

  • Lower back pain and cramping

  • Cervical changes (dilation and effacement)

When to go to the hospital or birth center:

  • Contractions every 5 minutes (lasting 1 minute) for at least 1 hour (the 5-1-1 rule)

  • Water breaks

  • Vaginal bleeding

  • Decreased fetal movement or unusual symptoms
