Proteases are enzymes that break down proteins and have been studied for their potential role in the management of skin abrasions and wound healing. Their use is primarily justified by scientific evidence rather than tradition. Proteases are applied in topical formulations, such as enzymatic debriding agents, to support the removal of necrotic tissue and cellular debris from wound surfaces, including abrasions. This process, known as debridement, facilitates the natural healing process by clearing obstacles to tissue regeneration and reducing the risk of infection.

Several clinical studies have demonstrated the effectiveness of protease-containing preparations (such as papain, trypsin, and bromelain) in accelerating wound healing and improving wound bed conditions. For instance, randomized controlled trials and case series have shown that enzymatic debridement with proteases is safe and effective for superficial wounds, helping to promote cleaner wounds and faster epithelialization compared to conventional treatments. Proteases may also modulate the wound environment by influencing inflammatory mediators and supporting the breakdown of excess fibrin, which can impede healing.

However, the overall quality of evidence is moderate. While protease-based debriding agents are widely accepted in clinical practice for chronic wounds and ulcers, fewer high-quality studies specifically address their use in minor abrasions. Thus, their use is scientifically validated, especially in more complex wounds, but the strength of evidence for routine use in simple abrasions is less robust, meriting a rating of 3 out of 5.
