Shikonin is a naphthoquinone compound derived primarily from the roots of Lithospermum erythrorhizon (commonly known as "Zicao" or "purple gromwell"). Its use in the treatment of abscesses is primarily rooted in traditional Chinese medicine (TCM). In TCM, preparations containing Shikonin have been applied topically or included in herbal formulas to "clear heat," "cool the blood," and promote the resolution of abscesses, boils, and other skin lesions characterized by redness, swelling, and pus formation.

Historically, Zicao has been valued for its purported abilities to reduce inflammation and promote tissue healing, and such properties are described in classical Chinese medical texts. The use of Shikonin-rich ointments for abscesses is common practice in many East Asian cultures, and such topical preparations remain widely available.

From a scientific perspective, there is some in vitro and animal evidence indicating that Shikonin possesses antibacterial, anti-inflammatory, and wound-healing activities. These findings provide a plausible rationale for its traditional use. However, there is a lack of high-quality, controlled clinical trials directly evaluating the efficacy of Shikonin or Zicao-based preparations in the treatment of abscesses in humans. Thus, while the traditional use is strong and supported by preliminary pharmacological data, definitive scientific validation is limited at this time.
