Alpha-terpineol is a naturally occurring monoterpene alcohol found in a variety of essential oils, including pine and tea tree oil. There is some scientific evidence suggesting alpha-terpineol has antimicrobial and anti-inflammatory properties, both of which are relevant to the pathogenesis of acne. In vitro studies have shown that alpha-terpineol can inhibit the growth of Cutibacterium acnes (formerly Propionibacterium acnes), the bacterium implicated in acne development. Additionally, its anti-inflammatory action may help reduce the redness and swelling associated with acne lesions. However, most of the available research is preliminary, conducted either in vitro or in animal models, and there is a lack of robust clinical trials specifically evaluating the efficacy of alpha-terpineol for treating acne in humans. Alpha-terpineol is sometimes included in topical formulations, especially as a component of tea tree oil, which does have more substantial evidence for use against acne. Ultimately, while the mechanistic rationale exists and laboratory studies are encouraging, clinical evidence remains limited. Thus, the scientific basis for using alpha-terpineol in acne is present but not strong, justifying a moderate evidence rating.
