Andrographolide is a diterpenoid lactone derived from Andrographis paniculata, a plant traditionally used in Asian medicine. While much of its use in folk medicine is targeted at inflammatory conditions and infections, some recent scientific studies have investigated its potential role in managing acne. Acne is a multifactorial skin disorder, characterized by inflammation, bacterial colonization (notably Cutibacterium acnes), and excessive sebum production.

Preclinical studies have shown that andrographolide exhibits anti-inflammatory and antibacterial activities. Specifically, in vitro research has demonstrated inhibitory effects of andrographolide on C. acnes, the main bacterium involved in acne pathogenesis. Additionally, andrographolide has been found to suppress pro-inflammatory cytokines such as TNF-α and IL-6, both of which are implicated in acne inflammation.

However, direct evidence from clinical trials evaluating andrographolide (or A. paniculata extracts) for acne treatment in humans is limited. Most data come from laboratory or animal studies, and only a few small-scale human studies or case reports suggest possible benefit. As a result, while there is a plausible scientific rationale and some preclinical support, the clinical evidence remains weak and insufficient to recommend its routine use for acne at this time. More robust randomized controlled trials in humans are needed to determine its efficacy and safety for this indication.
