Black cumin (Nigella sativa), and particularly its active constituent thymoquinone, has been used in traditional medicine for various skin conditions, including acne. However, in recent years, scientific studies have begun to substantiate some of its purported benefits for acne treatment. Several small-scale clinical trials and laboratory studies have indicated that black cumin seed oil has anti-inflammatory, antimicrobial, and antioxidant properties, all of which are relevant to acne pathogenesis. One randomized controlled trial published in 2010 found that a topical gel containing Nigella sativa extract significantly reduced acne lesions in patients compared to a placebo. Another split-face clinical study compared 20% black seed oil topical application with benzoyl peroxide, a standard acne treatment, and found comparable improvements in acne severity with fewer side effects in the black cumin group.
The proposed mechanisms include inhibition of Propionibacterium acnes (now Cutibacterium acnes), reduction of skin inflammation, and modulation of sebum production. While the overall body of evidence is still limited and consists mostly of small trials and preclinical studies, the results are promising and suggest real therapeutic potential. Larger, high-quality randomized controlled trials are needed to confirm efficacy and safety. In summary, the use of black cumin for acne is supported by both traditional use and emerging scientific evidence, earning a moderate evidence score.
