Lithospermum erythrorhizon, commonly known as purple gromwell or "Zi Cao" in traditional Chinese medicine (TCM), has a long history of use for various skin conditions, including acne. In TCM, the root of this plant is used for its purported "heat-clearing" and "detoxifying" properties, which are believed to help treat inflammatory skin disorders such as boils, carbuncles, and acne. The main active components, shikonin and its derivatives, have demonstrated anti-inflammatory and antimicrobial properties in laboratory studies, but robust clinical evidence in humans is lacking.

There are some in vitro and animal studies suggesting that shikonin can inhibit the growth of Propionibacterium acnes (now known as Cutibacterium acnes) and reduce skin inflammation. However, well-designed human clinical trials evaluating the efficacy and safety of Lithospermum erythrorhizon for acne are absent. Its use for acne is therefore primarily supported by traditional usage and some preliminary pharmacological studies, but not by high-quality clinical evidence. As such, the strength of evidence is moderate from a traditional medicine perspective but limited from a scientific standpoint.

In summary, Lithospermum erythrorhizon is traditionally used for acne in East Asian medicine, with some laboratory data supporting its anti-inflammatory and antibacterial actions. However, clinical validation remains insufficient.
