Scrophularia root (commonly known as Chinese figwort or Xuan Shen in Traditional Chinese Medicine, TCM) is primarily justified for use in acne by tradition rather than by robust scientific evidence. In TCM, Scrophularia root is considered to have "heat-clearing" and "detoxifying" properties, making it a popular ingredient in herbal formulas for skin conditions associated with "heat" and "toxicity," such as acne, boils, and other pustular skin eruptions. Historical texts and modern TCM practitioners often recommend Scrophularia root as part of multi-herb combinations to reduce inflammation, promote detoxification, and relieve swelling in acne-prone individuals.

However, the direct scientific evidence supporting the use of Scrophularia root for acne is limited. Some in vitro and animal studies suggest that Scrophularia species possess anti-inflammatory and antioxidant properties, which could theoretically benefit acne, an inflammatory skin disorder. However, clinical trials in humans specifically investigating Scrophularia root for acne are lacking.
