Sophora species, particularly Sophora flavescens (commonly known as Ku Shen), have a long history of use in traditional Chinese medicine (TCM) for various skin conditions, including acne. In TCM, Sophora root is believed to "clear heat" and "dry dampness," properties thought to counteract the pathogenic factors underlying acne. Traditional herbal formulas containing Sophora are often prescribed for their supposed anti-inflammatory and antibacterial effects, and topical preparations are also common.

Scientific validation for these uses is limited but not entirely absent. Some in vitro and animal studies indicate that extracts of Sophora flavescens possess antibacterial activity against Propionibacterium acnes (now Cutibacterium acnes), the bacterium involved in acne pathogenesis, and may exhibit anti-inflammatory effects. Active compounds such as matrine and oxymatrine have been identified as responsible for some of these actions. However, high-quality clinical trials in humans specifically evaluating Sophora for acne treatment are lacking, and much of the available evidence comes from traditional use or small, lower-quality studies. Thus, while there is a strong traditional basis and some preliminary scientific rationale, robust clinical evidence is insufficient at this time.
