Vitamin D has garnered interest in dermatology due to its immunomodulatory and anti-inflammatory properties, both of which are mechanisms implicated in the pathogenesis of acne. Several observational studies have reported lower serum levels of vitamin D in individuals with acne compared to controls. For example, a 2016 study in Dermato-Endocrinology observed that vitamin D deficiency was more prevalent among patients with acne vulgaris, and those with more severe acne had lower vitamin D levels. Some small interventional studies have also explored vitamin D supplementation as an adjunct therapy for acne, with mixed results; a 2016 randomized controlled trial in Cutaneous and Ocular Toxicology found that vitamin D supplementation led to a modest improvement in inflammatory acne lesions over eight weeks, though the sample size was small. Despite these findings, the evidence remains limited and somewhat inconsistent. Most available studies are observational or involve small sample sizes and short durations. No large-scale, high-quality randomized controlled trials have definitively established vitamin D as an effective treatment for acne. Thus, while there is a plausible biological rationale and some preliminary clinical data suggesting a potential benefit, current evidence does not firmly support routine vitamin D supplementation for acne treatment beyond correcting a deficiency. Vitamin D is not a first-line therapy for acne, but may be considered in those with proven deficiency or as part of a broader holistic approach.
