Serratiopeptidase, also known as serrapeptase, is a proteolytic enzyme that has been used in various countries for its anti-inflammatory, anti-edemic, and fibrinolytic properties. Its use in the treatment of adenitis (inflammation of lymph nodes) is primarily based on its general anti-inflammatory effects, which might help reduce swelling, pain, and edema associated with inflamed lymph nodes, regardless of the underlying cause (such as bacterial or viral infections). Several clinical studies and reviews, mainly from Japan and Europe, have reported that serratiopeptidase can reduce inflammation and facilitate the drainage of pus or exudates in conditions associated with localized inflammation and edema, such as sinusitis, mastitis, and post-surgical swelling. However, direct high-quality evidence specific to adenitis is lacking; most clinical studies assess related conditions or use serratiopeptidase as an adjunct to antibiotics or standard anti-inflammatory medications. In these studies, the enzyme appears to accelerate symptom resolution and reduce tissue swelling. The hypothesized mechanism is the enzymatic breakdown of inflammatory mediators and fibrin, promoting faster resolution of inflammatory processes. While there is a scientific rationale, the evidence is indirect and of moderate quality, with few placebo-controlled trials in adenitis specifically. Consequently, serratiopeptidase is sometimes used for adenitis in clinical practice, but its use is best considered as adjunctive to standard therapy rather than a primary treatment.
