Adrenal extract has a long history of use in traditional and early 20th-century Western medicine as a means to "support" adrenal function, particularly the adrenal cortex. It was commonly employed before synthetic corticosteroids became available, and proponents claimed it could help with fatigue, Addison’s disease, and various stress-related conditions. However, these claims were based on anecdotal reports and early medical practices rather than robust clinical trials. Traditional naturopathic and alternative health practitioners have also recommended adrenal glandulars (including adrenal extract) as part of protocols to address so-called "adrenal fatigue," a condition not formally recognized by mainstream medicine.

Scientific support for the use of adrenal extract is minimal. Modern research has not demonstrated that orally administered adrenal extracts effectively support adrenal cortex function or treat adrenal insufficiency. Furthermore, concerns about safety, standardization, and potential contamination have limited its use in evidence-based medicine. Regulatory agencies such as the FDA do not approve adrenal extract for any medical indication.
