Vitamin B5, also known as pantothenic acid, is often included in supplements marketed for "adrenal support." This use is rooted primarily in traditional and naturopathic practice rather than robust scientific evidence. Historically, pantothenic acid has been recognized as essential for the synthesis of coenzyme A, which plays a role in energy metabolism and the synthesis of steroid hormones produced by the adrenal cortex. Early animal studies from the mid-20th century showed that severe pantothenic acid deficiency could lead to adrenal atrophy and impaired adrenal function in rats. These findings led to the belief that B5 might support adrenal health in humans.

However, direct clinical evidence in humans to support the use of vitamin B5 for adrenal cortex function, particularly in the context of "adrenal fatigue" or stress adaptation, is lacking. The adrenal glands require several nutrients for optimal function, but there is no substantial human research indicating that supplementation with vitamin B5 beyond the recommended daily allowance provides any benefit for adrenal health. Thus, while traditional and naturopathic practitioners may recommend B5 for adrenal support, this is not strongly supported by modern scientific studies.
