Rhodiola sacra, closely related to Rhodiola rosea and sometimes considered the same species or a subspecies, has been used in traditional Chinese and Tibetan medicine. Historically, it has been employed as an adaptogen—a substance believed to help the body resist various stressors—and as a tonic to improve energy and endurance. Traditional uses include support for fatigue, improved mental performance, and enhancing the body's resilience to stress, which indirectly relates to supporting the adrenal glands, since these glands play a central role in the body's stress response.

However, the majority of scientific studies have focused on Rhodiola rosea rather than Rhodiola sacra specifically. Available evidence from studies on Rhodiola rosea suggests potential benefits in reducing fatigue and supporting the body's response to stress, but direct evidence for adrenal gland function is limited. There is a lack of robust clinical trials directly demonstrating that Rhodiola sacra supports adrenal health in humans.
