Vitamin B5, known as pantothenic acid (and in supplement form as calcium pantothenate or pantethine), plays a critical role in the synthesis of coenzyme A, which is essential for fatty acid metabolism and energy production. The adrenal glands require these metabolic pathways for hormone synthesis, particularly for producing corticosteroids such as cortisol. Animal studies from the mid-20th century demonstrated that severe pantothenic acid deficiency could lead to impaired adrenal function and decreased steroid hormone synthesis. Some early research also suggested that B5 supplementation could help support adrenal function under stress conditions. However, these studies were small and often outdated, with limited direct translation to human health or clinical contexts.

In humans, overt deficiency of Vitamin B5 is rare and typically only occurs in cases of severe malnutrition. Most people consuming a normal diet receive adequate amounts for normal adrenal function. There is a lack of high-quality, contemporary clinical trials directly demonstrating that supplemental Vitamin B5 or pantethine provides measurable benefits to adrenal health in otherwise healthy populations. Its inclusion in adrenal support formulas is therefore based on its biochemical necessity and supportive animal data rather than robust clinical trials in humans. Thus, while there is a scientific rationale for its importance in adrenal physiology, the evidence that supplementation supports adrenal health in the absence of deficiency is limited.
