Achyranthes, particularly Achyranthes aspera, has a long-standing presence in traditional medicine systems such as Ayurveda, Unani, and Traditional Chinese Medicine. In these traditions, it is sometimes used to support uterine health and promote the expulsion of retained products after childbirth, which is believed to help alleviate afterbirth pain (the cramping discomfort following delivery, often associated with uterine involution). Texts like the Charaka Samhita and Sushruta Samhita reference the use of Achyranthes for "uterine cleansing" and as an emmenagogue. In folk medicine, decoctions or infusions of the plant are used postnatally for their purported uterotonic effects. However, direct scientific validation for Achyranthes specifically treating afterbirth pain is scarce. Some phytochemical studies indicate that Achyranthes aspera contains saponins and alkaloids that may have uterotonic or muscle-stimulating properties, but these findings are mainly from in vitro or animal studies and do not directly address postpartum pain in humans. No clinical trials have evaluated its efficacy or safety for afterbirth pain. Thus, the use of Achyranthes for this indication is justified primarily by tradition, with limited preclinical evidence suggesting a plausible mechanism but lacking robust scientific validation.
