Benfotiamine is a synthetic derivative of thiamine (vitamin B1) designed to increase oral bioavailability. Its potential use in Alzheimer's disease (AD) is based on the hypothesis that thiamine deficiency and impaired glucose metabolism contribute to AD pathology. Preclinical studies have shown that benfotiamine can reduce pathological markers such as amyloid-beta deposition and tau phosphorylation in animal models of Alzheimer's. These effects are thought to be related to improved glucose metabolism, reduction in oxidative stress, and lowering of advanced glycation end-products (AGEs), all of which are implicated in neurodegeneration.
A small number of clinical studies have investigated benfotiamine in Alzheimer's patients. The most notable is a 2020 randomized, double-blind, placebo-controlled pilot study (Pan et al., 2020, JAMA Network Open), which found that benfotiamine supplementation (300 mg/day for 12 months) was safe and associated with slower cognitive decline compared to placebo. However, the study was small (n=70), and larger trials are needed to confirm these findings.
While these preliminary results are promising and suggest a scientific rationale for benfotiamine use in AD, the evidence is still limited and not sufficient to recommend it as a standard treatment. The evidence rating is therefore 2 out of 5, reflecting early clinical support but a need for more robust data.
