Beta Hydroxybutyrate (BHB) is a ketone body produced naturally in the liver during periods of low carbohydrate intake or fasting. Its use in supporting or treating Alzheimer’s Disease (AD) is justified primarily by emerging scientific interest, not tradition. The rationale centers on the observation that brains affected by AD often exhibit impaired glucose metabolism, leading to energy deficits. BHB can serve as an alternative energy substrate for neurons, bypassing some of the metabolic roadblocks associated with glucose.

Preclinical studies in animal models of AD have shown that increasing ketone bodies, including BHB, can improve cognitive function and reduce amyloid-beta pathology. A handful of small-scale human studies and pilot clinical trials have explored the effects of ketogenic diets or exogenous ketones (such as BHB salts or esters) on cognitive performance in patients with mild cognitive impairment or early AD. Some of these studies report modest improvements in cognition, particularly in those with metabolic impairments. However, the overall evidence remains preliminary, with small sample sizes and short durations.

To date, there is no robust, large-scale, long-term clinical trial data confirming the efficacy of BHB supplementation in AD. Nevertheless, the mechanistic rationale and early clinical findings have generated scientific interest, warranting further investigation. Thus, the evidence supporting BHB's use in AD can be rated as modest but not conclusive.
