Cannabinoids, the active compounds found in cannabis, have been investigated for their potential role in supporting or treating Alzheimer's disease (AD). The scientific interest is based on preclinical studies that suggest cannabinoids may exert neuroprotective, anti-inflammatory, and anti-oxidative effects. Laboratory and animal studies have shown that cannabinoids like THC and CBD may reduce beta-amyloid plaque accumulation, promote neurogenesis, and modulate neuroinflammation, all of which are implicated in AD pathology. For example, some studies have demonstrated that THC can inhibit the aggregation of beta-amyloid proteins and decrease neuron death in cell cultures. Additionally, animal models suggest cannabinoids may improve behavioral symptoms such as agitation and aggression. However, the clinical evidence in humans is very limited and mostly consists of small-scale or preliminary trials. These studies have primarily focused on behavioral symptoms rather than disease modification, with modest benefits and notable side effects. Larger, well-controlled clinical trials are lacking, and cannabinoids are not currently approved as a treatment for Alzheimer's disease by major regulatory agencies. Therefore, while there is a scientific rationale and preliminary evidence, the overall quality and quantity of clinical evidence supporting cannabinoid use in Alzheimer's disease is limited, and its use remains investigational.
