D-alpha tocopherol, a natural form of vitamin E, has been investigated for its potential role in supporting or treating Alzheimer's Disease (AD) due to its antioxidant properties. Oxidative stress is implicated in the pathogenesis of AD, and vitamin E is known to neutralize free radicals, potentially reducing neuronal damage. Some clinical trials have suggested modest benefits: the pivotal Petersen et al., 2005 study (published in NEJM) showed that high-dose vitamin E (2000 IU/day) slowed functional decline in patients with moderate Alzheimer's, compared to placebo. However, results have been inconsistent—other large studies, such as the Alzheimer's Disease Cooperative Study, found no significant cognitive benefit, and some meta-analyses raise concerns about high-dose vitamin E and all-cause mortality.

Current guidelines do not recommend vitamin E as a standard therapy for Alzheimer's, but it may be considered as an adjunct in certain cases, especially for mild to moderate forms. Importantly, most studies used high doses, which may carry risks like increased bleeding or cardiovascular events. In summary, while there is some scientific validation for d-alpha tocopherol’s use in Alzheimer's, the evidence is mixed and generally not strong, justifying a moderate evidence rating of 2/5.
