Eicosapentaenoic Acid (EPA) is an omega-3 polyunsaturated fatty acid found in fish oil and some algae. Its use in supporting or treating Alzheimer's Disease (AD) is primarily based on scientific investigation rather than traditional medicine. The rationale for EPA use involves its anti-inflammatory properties, as neuroinflammation is implicated in the pathogenesis of Alzheimer's Disease. Additionally, EPA, along with docosahexaenoic acid (DHA), is thought to support neuronal membrane fluidity and synaptic function, which may be compromised in AD.

Clinical evidence, however, is mixed and overall not robust. Some epidemiological studies have linked higher dietary intake of omega-3 fatty acids with reduced risk of cognitive decline and dementia. Interventional studies and randomized controlled trials (RCTs) specifically examining EPA (alone or in combination with DHA) in AD patients have yielded inconsistent results. A few small-scale trials have suggested modest cognitive benefits, particularly in individuals with mild cognitive impairment or very early-stage AD, but larger and more rigorous RCTs have failed to demonstrate significant clinical benefit in moderate or advanced Alzheimer's Disease. The most recent meta-analyses conclude that there is limited evidence for a meaningful effect of EPA supplementation on cognitive outcomes in AD.

Therefore, while there is a scientific basis for investigating EPA in AD, the clinical evidence supporting its use is weak to moderate at best, justifying a low evidence rating.
