Fish, particularly fatty fish such as salmon, mackerel, sardines, and anchovies, is often recommended for brain health due to its high content of omega-3 polyunsaturated fatty acids (especially EPA and DHA). Numerous epidemiological studies have associated higher consumption of fish and higher blood levels of omega-3 fatty acids with a reduced risk of cognitive decline and Alzheimer’s disease. The biological plausibility stems from the role of omega-3s in neuronal function, anti-inflammatory effects, and the maintenance of cell membrane integrity in the brain. Randomized controlled trials, however, have shown mixed results. Some trials report modest cognitive benefits or slowed decline in subjects with mild cognitive impairment or at risk for dementia, while others show no significant effect, especially in those already diagnosed with Alzheimer’s disease. The American Academy of Neurology and other medical groups acknowledge that omega-3 supplementation may be beneficial in preventing cognitive decline, but do not endorse it as a treatment for established Alzheimer’s disease. Shellfish, like oysters and clams, are less studied but are rich in nutrients like vitamin B12 and zinc, which are also important for neurological health. Overall, while there is some scientific rationale and observational support for fish in Alzheimer’s prevention, the clinical evidence is moderate and not conclusive for treatment.
