Ginkgo flavonglycosides, derived from the leaves of the Ginkgo biloba tree, are commonly used in standardized extracts (such as EGb 761) to support cognitive function, including in Alzheimer's disease (AD) and other forms of dementia. Several randomized controlled trials and meta-analyses have investigated their efficacy. The proposed mechanisms include antioxidant effects, improved cerebral blood flow, and modulation of neurotransmitter activity, which could theoretically benefit neurodegenerative conditions.
Clinical evidence for Ginkgo’s effectiveness in Alzheimer’s disease is mixed. Some studies have demonstrated modest improvements in cognitive function and activities of daily living compared to placebo, particularly in mild-to-moderate dementia. For example, a 2007 Cochrane review and a 2016 meta-analysis found statistically significant but clinically modest benefits. However, other large, well-designed trials (e.g., the Ginkgo Evaluation of Memory [GEM] study, 2008) did not find Ginkgo biloba extract to be superior to placebo in preventing or treating dementia or Alzheimer's disease.
Major guidelines do not recommend Ginkgo as a first-line treatment for Alzheimer's due to the inconsistency and modesty of the results, as well as concerns about safety, particularly bleeding risk. Overall, there is some scientific evidence supporting the use of Ginkgo flavonglycosides in AD, but the quality and strength of the evidence are moderate at best, warranting a rating of 2 out of 5.
