Ginkgoheteroside refers to a group of flavonoid glycosides found in the leaves of Ginkgo biloba. Extracts from Ginkgo biloba, standardised to contain specific amounts of ginkgoheterosides (as flavone glycosides) and terpene lactones, have been studied for their potential effects in Alzheimer’s disease (AD). The rationale for use is based on proposed antioxidant, anti-inflammatory, and neuroprotective properties, as well as possible enhancement of cerebral blood flow.

Several clinical trials and meta-analyses have tested Ginkgo biloba extract (not ginkgoheteroside in isolation) in patients with dementia and Alzheimer’s disease. Results generally show small, inconsistent benefits for cognitive function and activities of daily living compared to placebo. For example, a 2016 Cochrane review found that some studies suggested mild improvement in cognition, but the overall evidence quality was low to moderate, and clinical significance remains uncertain. Guidelines from major neurological organizations do not recommend Ginkgo biloba as a primary treatment due to limited and inconsistent efficacy and methodological limitations in studies.

Importantly, while Ginkgo biloba extract is widely used in traditional and complementary medicine for cognitive disorders, the specific role of ginkgoheterosides is not well defined. Most evidence refers to the standardized extract as a whole, not individual components. Therefore, the scientific support for ginkgoheterosides specifically in Alzheimer’s is limited (rated 2/5), and benefits, if any, are likely modest.
