Ginsenosides are the primary active compounds found in ginseng, particularly Panax ginseng, and have been investigated for their neuroprotective effects, including potential benefits in Alzheimer's Disease (AD). Scientific interest stems from preclinical studies showing that certain ginsenosides (such as Rg1, Rb1, and Rd) can modulate neurotransmitter systems, reduce beta-amyloid aggregation, and exert anti-inflammatory and antioxidant effects, all relevant to AD pathology. Several animal studies have demonstrated improvements in learning and memory following ginsenoside administration in AD models. In vitro studies also indicate that ginsenosides may protect neuronal cells from beta-amyloid-induced toxicity. Human evidence, however, remains limited and of low quality. A few small clinical trials and open-label studies have suggested possible cognitive benefits of ginseng or ginsenosides in patients with mild to moderate AD, but these studies often lack rigorous controls, have small sample sizes, or use mixed preparations, making it difficult to attribute effects to ginsenosides specifically. There is currently no high-quality, large-scale randomized controlled trial conclusively demonstrating efficacy for Alzheimer's Disease. In summary, ginsenosides are used to support Alzheimer's Disease based on promising preclinical data and some preliminary clinical indications, but robust scientific validation is lacking. More well-designed human trials are needed to confirm their therapeutic potential for this condition.
