Magnesium threonate is a compound developed to increase magnesium levels in the brain more effectively than other magnesium salts. Its use in supporting or treating Alzheimer’s Disease is based primarily on preclinical and limited human data. Animal studies, most notably by Slutsky et al. (2010), showed that magnesium threonate supplementation increased synaptic density and improved learning and memory in aged rats. These findings have led to speculation that it may help with cognitive decline in humans, including Alzheimer’s Disease. A small pilot human study (Liu et al., 2016) found that magnesium threonate supplementation in older adults with cognitive impairment improved certain measures of executive function and memory over a 12-week period compared to placebo. However, the study was small (n=44), short in duration, and did not specifically enroll patients with diagnosed Alzheimer’s Disease. There are currently no large, randomized, placebo-controlled trials demonstrating that magnesium threonate slows progression or improves symptoms of Alzheimer’s Disease in humans. Despite promising mechanistic and animal data, the current level of clinical evidence is limited and insufficient to recommend magnesium threonate as a treatment for Alzheimer’s Disease. It is not included in clinical guidelines and should not be considered a proven therapy. Ongoing research may clarify its role in the future.
