Nerve Growth Factor (NGF) is a neurotrophin essential for the survival and maintenance of certain neurons in the brain, particularly basal forebrain cholinergic neurons that are crucial for memory and cognitive function. In Alzheimer's Disease (AD), these neurons degenerate, and levels of NGF and its signaling are often disrupted.

Research interest in NGF as a potential treatment for AD is based on preclinical studies showing that NGF can prevent or slow the degeneration of cholinergic neurons in animal models. In the 1990s and 2000s, several small clinical trials and case studies explored NGF administration—via intracerebral grafts, gene therapy, or direct infusion—into the brains of AD patients. Some studies reported modest improvements or stabilization of cognitive decline, while others found limited efficacy and significant safety or delivery challenges (e.g., pain, weight loss, complications from surgical delivery).

The overall scientific evidence supporting NGF for AD remains preliminary. The majority of human studies have been small and open-label, with no large, definitive, placebo-controlled trials demonstrating significant clinical benefits. Key obstacles include difficulty in delivering NGF to the brain and potential side effects.
