Olive oil, particularly extra virgin olive oil (EVOO), has gained scientific interest for its potential role in supporting brain health and possibly reducing risk factors associated with Alzheimer’s Disease (AD). The Mediterranean diet, which is rich in olive oil, is epidemiologically linked with lower rates of cognitive decline and AD. Preclinical studies in animal models have shown that EVOO can reduce amyloid-beta plaque formation, neuroinflammation, and oxidative stress, all of which are implicated in AD pathogenesis. Notably, studies such as the PREDIMED trial and its sub-analyses reported that adherence to a Mediterranean diet supplemented with olive oil was associated with improved cognitive function compared to control diets. However, evidence from human clinical trials specifically targeting AD prevention or progression remains limited and mostly indirect. Most clinical studies are observational or involve dietary patterns rather than isolated olive oil supplementation. The bioactive compounds in olive oil, such as polyphenols (e.g., oleocanthal and hydroxytyrosol), are thought to exert neuroprotective effects, but definitive large-scale, randomized controlled trials in humans with established AD are lacking. In summary, while there is some scientific rationale and observational evidence that olive oil consumption may support brain health and potentially lower AD risk, the direct evidence for its use as a treatment or preventive agent for Alzheimer’s Disease in humans is modest.
