Phytocannabinoids, such as cannabidiol (CBD) and tetrahydrocannabinol (THC), have been investigated for their potential to support or treat Alzheimer's disease (AD), primarily based on preclinical and early clinical data. The rationale stems from the endocannabinoid system's involvement in neuroprotection, neuroinflammation, and neurotransmission, all of which are implicated in AD pathology. Animal studies and in vitro models have shown that certain phytocannabinoids may reduce neuroinflammation, oxidative stress, and amyloid-beta toxicity—key features of Alzheimer's disease. For example, CBD has demonstrated the ability to decrease reactive gliosis and neuroinflammatory markers in mouse models, while THC has been shown to inhibit acetylcholinesterase and reduce amyloid-beta aggregation in laboratory studies.

However, high-quality human studies are limited. A few small-scale clinical trials have examined the effects of cannabinoid-based medications on behavioral symptoms in AD patients, such as agitation and sleep disturbances, with mixed or modestly positive outcomes. There is currently no robust clinical evidence that phytocannabinoids can halt or reverse the underlying neurodegenerative processes of Alzheimer's disease. Most medical guidelines do not recommend cannabinoids as a standard treatment for AD, emphasizing the need for larger, well-controlled human trials. As a result, while there is a scientific basis for ongoing research and some preliminary evidence of benefit regarding symptom management, the overall level of evidence for phytocannabinoids in treating Alzheimer's disease is currently low (rated 2 out of 5).
