Polyunsaturated fats (PUFAs), particularly omega-3 fatty acids like docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA), have been investigated for their potential role in supporting cognitive function and slowing the progression of Alzheimer's disease (AD). The scientific rationale stems from the role of PUFAs in maintaining neuronal membrane integrity, modulating inflammation, and supporting synaptic function—all of which are relevant to neurodegeneration in AD.
Epidemiological studies suggest that higher dietary intake of omega-3 PUFAs is associated with a reduced risk of cognitive decline and Alzheimer's disease. Some observational studies have shown that individuals with higher plasma levels of DHA have a lower risk of developing AD. However, randomized controlled trials (RCTs) on PUFA supplementation in people with existing AD have largely produced mixed or modest results. Some trials report small benefits in slowing cognitive decline, especially in those with mild cognitive impairment or in early stages of the disease, but others find no significant effect.
A 2020 Cochrane review concluded that omega-3 supplementation does not significantly benefit cognitive function in people with mild to moderate AD, but may have a potential preventive effect before disease onset. Overall, the evidence supports a biological plausibility and some potential benefit, but does not strongly validate routine PUFA use for treating established Alzheimer's disease.
