Potassium Beta-Hydroxybutyrate (BHB) is a ketone body and exogenous ketone supplement often marketed for cognitive enhancement, including in Alzheimer's Disease (AD). The rationale behind its use in AD is based on the observation that the brains of individuals with Alzheimer’s experience impaired glucose metabolism, sometimes described as "Type 3 diabetes." Ketones, like BHB, can serve as an alternative energy source for neurons when glucose utilization is compromised.
Some small clinical studies and preclinical animal research have provided preliminary evidence that ketone bodies (via ketogenic diets or exogenous ketones like BHB) may improve cognitive function in patients with mild-to-moderate AD or mild cognitive impairment (MCI). For example, a 2018 phase II trial (Henderson et al.) using a ketogenic drink containing BHB showed modest cognitive improvements in participants with mild-to-moderate AD. Animal studies also suggest that ketones may reduce amyloid-beta accumulation and neuroinflammation.
However, the overall body of evidence remains limited: most studies are small, short-term, and often use ketogenic diets or mixed ketone formulations, not specifically Potassium BHB alone. There is currently insufficient high-quality, long-term clinical evidence to support routine use of Potassium BHB as a treatment for AD. Larger, well-controlled trials are required to determine efficacy, safety, and optimal dosing. Thus, while there is a plausible scientific rationale and some early clinical data, the evidence base is modest.
