Punicalagins are high-molecular-weight polyphenols found primarily in pomegranate (Punica granatum) and are known for their antioxidant and anti-inflammatory properties. Their potential use in supporting or treating Alzheimer's disease (AD) is based on preclinical scientific research rather than traditional medical use. Several in vitro and animal studies have suggested that punicalagins and their metabolites may help reduce neuroinflammation, oxidative stress, and amyloid-beta aggregation—key pathological features of Alzheimer’s disease. For instance, studies have shown that administration of pomegranate extracts rich in punicalagins can reduce amyloid plaque formation and improve cognitive performance in transgenic mouse models of AD. The neuroprotective effects are thought to be mediated through the modulation of signaling pathways involved in inflammation and oxidative damage.
However, there is a lack of robust clinical evidence in humans. To date, no large-scale randomized controlled trials have been conducted to directly assess the efficacy of punicalagins or pomegranate extracts in patients with Alzheimer's disease. The existing human studies are limited in number, often use whole pomegranate extract rather than isolated punicalagins, and primarily focus on cognitive support rather than disease modification. Therefore, while there is scientific interest and some supportive preclinical data, the evidence is still preliminary. More rigorous clinical trials are needed to confirm any potential therapeutic role of punicalagins in Alzheimer’s disease.
