Superoxide Dismutase (SOD) is an endogenous antioxidant enzyme that catalyzes the dismutation of superoxide radicals into oxygen and hydrogen peroxide, thereby protecting cells from oxidative stress. The rationale for investigating SOD in Alzheimer's Disease (AD) stems from evidence implicating oxidative stress in the pathogenesis of AD. Experimental studies have found increased markers of oxidative damage in AD brains and reduced antioxidant defense mechanisms, including altered SOD activity.

Preclinical studies in cell lines and animal models suggest that enhancing SOD activity can mitigate some pathological features of AD, such as amyloid-beta toxicity and neuronal loss. However, the translation of these findings to clinical benefit in humans remains unclear. Oral supplementation of SOD faces significant challenges, as the enzyme is typically degraded in the gastrointestinal tract, limiting its bioavailability. Few clinical trials have directly assessed SOD supplementation in AD patients, and available studies are small and methodologically limited. While some indirect evidence exists for the benefit of antioxidant therapies in neurodegenerative diseases, robust clinical evidence specifically supporting SOD supplementation in Alzheimer’s patients is lacking.

In summary, there is a scientific rationale and preclinical evidence supporting the potential role of SOD in AD, but clinical validation is currently insufficient. Thus, the evidence rating for its use in Alzheimer’s Disease is low (2/5), reflecting a need for well-designed human studies.
