Tauroursodeoxycholic acid (TUDCA) is a bile acid derivative that has gained interest for its potential neuroprotective properties in neurodegenerative disorders, including Alzheimer's disease (AD). The scientific rationale for its use is based primarily on preclinical studies. TUDCA has demonstrated the ability to reduce endoplasmic reticulum stress, inhibit apoptosis, and protect mitochondrial function in cellular and animal models of neurodegeneration. Several studies in transgenic mouse models of Alzheimer’s disease have shown that TUDCA administration can reduce amyloid-beta aggregation, decrease neuronal loss, and improve cognitive performance. For example, a 2004 study in Journal of Neuroscience reported that TUDCA prevented apoptosis and attenuated amyloid pathology in an AD mouse model.
Despite these promising preclinical findings, high-quality human clinical trials evaluating the efficacy and safety of TUDCA in Alzheimer's disease are lacking. No large-scale, peer-reviewed randomized controlled trials have been published as of 2024. As such, the evidence supporting its use in AD remains preliminary, and its clinical utility has not been established. Current scientific validation is therefore limited to animal and cell-based studies, warranting a modest evidence rating of 2 out of 5. Further research, particularly well-designed human trials, is necessary before TUDCA can be recommended as a treatment for Alzheimer’s disease.
