Tetrahydrocannabinol (THC), the main psychoactive compound in cannabis, has attracted research interest for its potential effects on Alzheimer's Disease (AD). Preclinical studies, especially in vitro and animal models, indicate that THC may reduce the accumulation of amyloid-beta plaques—a hallmark of AD—by inhibiting the enzyme acetylcholinesterase and reducing amyloid aggregation. THC has also been shown to have anti-inflammatory and neuroprotective properties, which could theoretically help slow neurodegeneration.
However, robust clinical evidence in humans is lacking. Most published human studies have small sample sizes and focus on symptomatic relief—such as agitation, aggression, and appetite loss in advanced AD—rather than disease modification. Some small trials report modest improvement in behavioral symptoms, but results are inconsistent, and side effects such as sedation, confusion, and increased fall risk are concerns, especially in elderly populations.
Major medical organizations generally do not recommend THC as a treatment for Alzheimer's Disease due to insufficient high-quality evidence. Current research is ongoing, but as of now, the scientific validation is limited, and the evidence supporting THC’s use in AD is weak to moderate at best.
