Uridine, a nucleoside involved in RNA synthesis and brain phospholipid metabolism, has been investigated as a potential adjunctive therapy for Alzheimer's Disease (AD). The scientific rationale stems from its role, along with choline and DHA, in the Kennedy pathway, which is essential for the synthesis of synaptic membranes. Preclinical studies have demonstrated that uridine supplementation can promote synaptogenesis and improve memory deficits in animal models of AD. Clinical research has primarily focused on combinations of uridine, choline, and DHA (such as in the medical food Souvenaid). Several randomized controlled trials have reported modest improvements in memory performance in patients with mild AD, though effects on global cognition and daily functioning have been inconsistent or minimal. Meta-analyses and systematic reviews generally characterize the evidence as preliminary or suggestive, with most benefits limited to early-stage disease and with combination therapy rather than uridine alone. Overall, while uridine's use is based on a plausible biological mechanism and some supportive clinical data, the strength of evidence remains limited (rated 2/5), and major guidelines do not recommend uridine as a standard treatment for AD.
