Vitamin E, including beta and delta tocopherols, has been studied for its potential role in supporting or treating Alzheimer's Disease (AD), primarily due to its antioxidant properties. Oxidative stress is implicated in the pathogenesis of AD, and vitamin E is known to neutralize free radicals, which has led to the hypothesis that it could slow cognitive decline or neurodegeneration. Most clinical research has focused on alpha-tocopherol, the main form of vitamin E in supplements, though beta and delta tocopherols are also natural components of the vitamin E family found in foods. A pivotal randomized controlled trial published in the New England Journal of Medicine in 1997 found that high-dose alpha-tocopherol slowed functional decline in patients with moderate AD. However, subsequent studies, such as the TEAM-AD VA Cooperative Randomized Trial (2014), showed only modest benefits, and meta-analyses have yielded mixed results, with some noting no significant cognitive improvement and potential safety concerns at high doses. Data on beta and delta tocopherols specifically are limited; most mechanistic and population studies do not differentiate among tocopherol forms. Current clinical guidelines do not recommend vitamin E supplementation as a standard therapy for AD due to inconsistent efficacy and potential risks. In summary, while there is some scientific rationale and limited clinical evidence for vitamin E's use in AD, the evidence is rated as low (2/5), particularly for beta and delta tocopherols, and does not currently support widespread clinical use for Alzheimer's Disease.
