Vitamin E (specifically alpha-tocopherol, but sometimes including beta-tocopherol as part of the tocopherol family) has been investigated for its potential role in supporting or treating Alzheimer’s Disease (AD) due to its antioxidant properties. The rationale is that oxidative stress is a known contributor to neuronal damage in AD, and antioxidants like vitamin E might protect brain cells from such damage. Some early observational studies and small clinical trials suggested a possible benefit in slowing functional decline in AD patients. A notable randomized controlled trial published in JAMA (2002) found that high-dose alpha-tocopherol (2000 IU/day) modestly delayed clinical progression in patients with moderate AD. However, subsequent larger studies and systematic reviews have shown inconsistent results, with some failing to show significant cognitive benefits. Additionally, concerns have been raised about potential adverse effects of high-dose vitamin E (e.g., increased mortality risk in some populations).

It is important to note that most research focuses on alpha-tocopherol, not beta-tocopherol. There is little direct clinical evidence for beta-tocopherol specifically in AD. Thus, while the use of vitamin E in Alzheimer’s has some scientific basis, the evidence is mixed and generally weak, with most major guidelines not recommending vitamin E as a standard treatment. The evidence rating reflects modest support based mainly on alpha-tocopherol data, with significant uncertainty and lack of consistent benefit in clinical outcomes.
