Vitamin E, particularly in the forms of mixed tocopherols and tocotrienols, has been investigated for its potential to support or treat Alzheimer's Disease (AD) due to its antioxidant properties. Oxidative stress is implicated in the pathogenesis of AD, and Vitamin E is a lipid-soluble antioxidant that can scavenge free radicals in neuronal membranes. Several randomized controlled trials and meta-analyses have examined the effects of Vitamin E supplementation in AD patients. The most notable is the 1997 Sano et al. study published in The New England Journal of Medicine, which found that high-dose alpha-tocopherol (2,000 IU/day) modestly delayed functional decline in patients with moderate AD. However, subsequent research has produced mixed results. Some studies observed no significant cognitive benefit, and concerns have been raised about possible increased mortality at high doses. The 2014 TEAM-AD VA Cooperative Randomized Trial reported a slower functional decline in mild-to-moderate AD with Vitamin E supplementation, but no effect on cognitive scores. There is limited research specifically on mixed tocopherols and tocotrienols, with most clinical data focused on alpha-tocopherol. Current clinical guidelines do not recommend Vitamin E as a primary treatment for AD due to insufficient evidence of significant benefit and potential risks. Overall, scientific support exists but is modest (evidence rating: 2/5), with further high-quality studies needed to clarify its role.
