Vitamin E has been investigated as a possible treatment for Alzheimer's Disease (AD) due to its antioxidant properties, which could theoretically help reduce oxidative stress implicated in neurodegeneration. Notably, a 1997 randomized controlled trial (Sano et al., NEJM) found that high-dose vitamin E (2000 IU/day) modestly delayed clinical progression in patients with moderate AD. More recently, the 2014 TEAM-AD VA Cooperative Randomized Trial (Dysken et al., JAMA) reported that vitamin E slowed functional decline in patients with mild to moderate AD compared to placebo, with a mean difference of 6.2 months in disease progression. However, no significant benefit was seen in combination with memantine, and results have not been universally replicated. Meta-analyses indicate mixed results, with some showing minor cognitive or functional benefits, but also raising concerns about all-cause mortality at high doses. Professional guidelines (e.g., American Academy of Neurology, 2020) note the modest benefit but also the lack of strong evidence to recommend routine use due to safety concerns and inconsistent findings.
