Estrogen is scientifically validated for the treatment of amenorrhea, especially when the underlying cause is hypoestrogenism, such as seen in hypothalamic amenorrhea, premature ovarian insufficiency, or after oophorectomy. Amenorrhea, defined as the absence of menstruation, can result from insufficient endogenous estrogen production. Estrogen therapy (often combined with a progestin) is a mainstay in the management of these forms of amenorrhea to induce withdrawal bleeding, maintain secondary sexual characteristics, and prevent long-term complications such as osteoporosis and cardiovascular disease. Numerous clinical guidelines, including those from the Endocrine Society and American College of Obstetricians and Gynecologists, support the use of estrogen replacement in women with amenorrhea due to hypoestrogenic states. Estrogen therapy restores the endometrial proliferation necessary for menstruation and addresses systemic effects of estrogen deficiency. Randomized controlled trials and observational studies have demonstrated the efficacy of estrogen in re-establishing menstrual cycles and improving bone health in this population. However, estrogen is not indicated for amenorrhea resulting from non-hormonal etiologies, such as structural uterine abnormalities or outflow tract disorders. In summary, the use of estrogen for amenorrhea due to estrogen deficiency is strongly supported by scientific evidence and clinical practice.
