Colocynth (Citrullus colocynthis), also known as bitter apple, has a long history of use in traditional medicine systems, particularly in Unani and Ayurvedic medicine. Its primary traditional applications have revolved around its strong purgative, anti-inflammatory, and analgesic properties. In the context of anal fistula and fissure, colocynth is sometimes included in traditional formulations aimed at reducing inflammation and pain, and promoting local healing. However, scientific validation for its efficacy in treating anal fistula or fissure is minimal to nonexistent. Most of the available literature describes its use for gastrointestinal complaints, constipation, and as an anti-inflammatory or antimicrobial agent in broad terms.
