Lodhtree (Symplocos racemosa), commonly known as Lodhra, is widely used in traditional Ayurvedic medicine in India. Its use for treating anorectal conditions such as anal fistula and fissure is primarily grounded in traditional practice rather than robust scientific validation. Ancient Ayurvedic texts describe Lodhtree as having astringent, anti-inflammatory, and wound-healing properties. It is often included in formulations (like ointments or decoctions) intended to promote healing of mucosal and skin wounds, and to reduce local inflammation and bleeding, which are relevant to conditions like anal fissure and fistula. The main rationale in Ayurveda is its shodhana (cleansing) and ropana (healing) effects, which are believed to aid the closure of chronic wounds. However, clinical trials or rigorous scientific studies specifically evaluating Lodhtree’s effectiveness in anal fistula or fissure are lacking. Some in vitro and animal studies suggest that Lodhtree has anti-inflammatory and antimicrobial properties, but these are not specific to anorectal pathologies. Thus, while its use is common in traditional settings and is sometimes included in polyherbal preparations for these conditions, there is currently limited high-quality evidence to support its effectiveness from a modern biomedical perspective.
