Synopsis

Source of validity: Traditional
Rating (out of 5): 2

Patrinia, especially Patrinia scabiosaefolia (commonly called Bai Jiang Cao in Traditional Chinese Medicine), has a long history of use in East Asian herbal medicine for the treatment of various infectious and inflammatory conditions, including anal fistula and anal fissure. In traditional practice, Patrinia is thought to possess "heat-clearing" and "detoxifying" properties, and is often included in herbal formulas aimed at reducing local infection, swelling, and promoting tissue healing in anorectal diseases. Several classic Chinese Materia Medica texts and clinical tradition cite its application for "sores" and "intestinal abscesses".

However, while there are case reports and small-scale clinical studies—mainly in Chinese medical literature—suggesting Patrinia-containing formulas may help reduce symptoms, these studies often lack rigorous controls, are of low methodological quality, and are rarely published in indexed, peer-reviewed journals. There is limited modern biomedical evidence directly supporting its efficacy for anal fistula or fissure, and no high-quality randomized controlled trials exist to date. Thus, the use of Patrinia in this context is better characterized as traditional, with modest supporting evidence primarily from historical use and a few preliminary or anecdotal studies.
