The rationale is that psyllium softens stool and increases its bulk, thereby reducing straining and trauma during defecation, which can help prevent further injury to the anal mucosa and promote healing of fissures. Several guidelines (e.g., American Society of Colon and Rectal Surgeons) recommend fiber supplementation, including psyllium, as first-line management for acute and chronic anal fissure. Some randomized controlled trials have shown that fiber supplementation can improve healing rates and decrease pain compared to placebo in anal fissure patients.
