Sangre de Grado (also known as Dragon's Blood) is a traditional remedy derived from the sap of the Croton lechleri tree, commonly used in South American folk medicine. Its application for anal fissures and fistulas is rooted primarily in traditional use rather than robust clinical evidence. The sap has long been used topically for wound healing, due to its purported antimicrobial, anti-inflammatory, and astringent properties. Historical accounts and ethnobotanical surveys document its use for various types of skin lesions, ulcers, and mucosal injuries, which extends to anorectal conditions like fissures and fistulas in traditional medicine practices in the Amazon. From a scientific perspective, a handful of in vitro and animal studies suggest that Sangre de Grado’s active compounds (notably proanthocyanidins and taspine) can promote wound healing and reduce inflammation. There is also some limited evidence that it may accelerate re-epithelialization and reduce pain when used topically. However, human clinical studies specifically investigating its efficacy for anal fissures or fistulas are lacking. The few clinical studies on Sangre de Grado focus more broadly on wound healing and gastrointestinal ulcers. In summary, while traditional use justifies its application for anal fissures and fistulas, scientific validation for these specific indications remains minimal and indirect. More rigorous clinical trials are needed to establish its safety and efficacy for these anorectal conditions.
