Vitex negundo (Nirgundi) has a long history of traditional use in Ayurvedic and other systems of traditional medicine for a variety of inflammatory and pain-related conditions, including those affecting the anorectal region such as anal fistula and fissure. Its leaves and oil preparations are applied topically or used as poultices to reduce pain, swelling, and promote healing. The rationale for its use is largely based on its reputed anti-inflammatory, analgesic, and wound-healing properties, as described in traditional texts and Ayurvedic pharmacopeias. However, while there is some preclinical evidence demonstrating anti-inflammatory and antimicrobial effects of Vitex negundo extracts, there is very limited clinical research specifically evaluating its efficacy in the treatment of anal fistula or fissure in humans. Most available evidence comes from anecdotal reports, traditional practice, and a few small, uncontrolled studies. Therefore, its use for these conditions is justified primarily by tradition rather than robust scientific validation, and the evidence supporting its effectiveness remains modest and preliminary.
