17beta-[{1-Ketoethyl}-androsta-1,4-diene-3-one,17a-ol] is the chemical name for oxandrolone, a synthetic anabolic steroid. Oxandrolone and other anabolic steroids have a scientifically validated history of use in the treatment of anemia, particularly anemia associated with chronic diseases, bone marrow failure, or following chemotherapy or radiation. The therapeutic rationale is that anabolic steroids stimulate erythropoiesis (red blood cell production) by increasing erythropoietin synthesis and enhancing the proliferation of erythroid progenitor cells in the bone marrow.

Clinical studies dating back to the 1960s and 1970s demonstrated that oxandrolone and related anabolic steroids, such as oxymetholone and nandrolone, can significantly increase hemoglobin and red cell mass in patients with various forms of anemia, including aplastic anemia, myelodysplastic syndromes, and anemia of chronic kidney disease. Oxandrolone is less androgenic than many anabolic steroids, making it preferable for children and women in some clinical contexts. The U.S. FDA approved oxandrolone for "adjunctive therapy to promote weight gain after weight loss following extensive surgery, chronic infections, or severe trauma," but it has also been used off-label for anemia.

However, the introduction of recombinant human erythropoietin and improvements in transfusion medicine have largely replaced anabolic steroids in the management of anemia. Nevertheless, the scientific evidence for oxandrolone and related compounds in stimulating erythropoiesis is robust, making its previous use for anemia scientifically justified.
